Pandora
(Oh, and feature request for the next iPod / iPhone: Allow behavioral smart playlists. I want to create two in particular: Songs I tend to skip, and songs I tend to turn up.)
Basically the Pandora approach right?

Yeah, Pandora does track what you skip. I don’t think they can track what you do with the volume.
In any case, the useful thing would be to have the flagged songs in a separate playlist I could review occasionally for song rating quality control.
